Please read these Terms of Service carefully before using MedicalRecords.in.
1. Acceptance of Terms
By accessing or using MedicalRecords.in ("Service"), you agree to be bound by these Terms of Service ("Terms"). If you disagree with any part of these terms, you may not access the Service.
These pre-release Terms apply to test and demonstration access. The identified contracting entity, effective date, commercial terms, and required consumer notices must be completed before a paid launch.
2. Description of Service
MedicalRecords.in provides:
• An individual adult’s source-record organization and dated timeline
• Patient-authored record labels, dates, questions, and visit-brief preparation
• Machine-draft document extraction kept private behind the doctor-release boundary
• Descriptive, patient-entered health measurements without diagnosis or advice
• Privacy preferences, recorded sharing events, and read-only history for legacy privacy requests
Medication-reminder automation, general doctor-patient communication, family-record access, new privacy-request intake, direct data export, and automatic erasure are not included in this pre-commercial release.
**Important Disclaimer**: Machine output is a draft and is not medical advice, diagnosis, or treatment. Patient-facing clinical interpretation is intended to require review by an eligible doctor. Availability of a doctor or review time is not guaranteed unless a separate written service commitment says so.
3. User Accounts
To use certain features, you must create an account. You agree to:
• Provide accurate and complete information
• Maintain the security of your password
• Accept responsibility for all activities under your account
• Notify us immediately of any unauthorized access
You must be at least 18 years old to create a pre-commercial account unless we provide a separate, approved parent or guardian flow.
4. Acceptable Use
You agree NOT to use the Service to:
• Upload content that violates any law or regulation
• Impersonate any person or entity
• Interfere with or disrupt the Service
• Attempt to gain unauthorized access to systems
• Use automated means to access the Service without permission
• Upload malicious code or harmful content
• Harass, abuse, or harm another person
• Use for any unlawful purpose
The operator may suspend or terminate test access that violates these terms.
5. Medical Disclaimer
**CRITICAL**: MedicalRecords.in is NOT a substitute for professional medical advice.
• Machine extraction can be incomplete or wrong and is kept as a draft
• Doctor review does not replace an examination or an established treating relationship
• Always verify medication information with your doctor or pharmacist
• Do not make medical decisions based solely on the Service
• Seek emergency medical help for serious symptoms
The Service is not an emergency service. If you believe someone may be in immediate danger, contact local emergency services or a qualified clinician directly.
6. Intellectual Property
Repository source is offered under its published open-source licence. Ownership and permitted use of product content, trademarks, and third-party material remain subject to the applicable licence and the final contracting entity’s approved terms.
You retain rights in content you upload. By uploading, you authorize the selected deployment to process the source record only for the enabled service purposes and applicable consent choices. Final commercial licence wording requires counsel review.
7. Payment Terms
Commercial plans and paid subscriptions are not represented as launched by these pre-release Terms. Before accepting payment, MedicalRecords.in must publish the applicable price, taxes, renewal terms, cancellation method, refund policy, service scope, doctor-review availability, and payment processor at the point of purchase.
8. Limitation of Liability
To the maximum extent permitted by law:
• The Service is provided "AS IS" without warranties of any kind
• We do not guarantee accuracy, completeness, or reliability
• We are not liable for any indirect, incidental, or consequential damages
• Any enforceable limitation is subject to applicable law and the final commercial agreement
Nothing in these Terms excludes rights or liability that cannot lawfully be excluded. This section requires counsel review before commercial launch.
9. Indemnification
You agree to defend and indemnify MedicalRecords.in from any claims, damages, or expenses arising from:
• Your use of the Service
• Your violation of these Terms
• Your violation of any third-party rights
• Content you submit to the Service
10. Termination
We may suspend test access when reasonably necessary for:
• Breach of these Terms
• Illegal activity
• Extended inactivity
Account closure, data access, and erasure require a verified operator process and the deployment’s approved retention obligations. No such intake process, fixed account-deletion period, or retention period is offered by these pre-release Terms. Provisions that should survive termination will remain in effect.
11. Governing Law
These pre-release Terms are intended to be governed by the laws of India, subject to mandatory consumer and other applicable protections. The contracting entity, registered address, dispute process, and competent jurisdiction must be inserted before commercial launch.
12. Changes to Terms
We may update these Terms. The final contracting entity must define the applicable notice channel, effective date, and acceptance process before commercial launch.
Where applicable law requires notice or affirmative acceptance, we will obtain it before revised terms take effect.
13. Contact Us
No operational legal-notice channel is published in this pre-commercial draft.
The contracting entity, postal address, legal-notice channel, and grievance/escalation contacts must be inserted and verified before commercial launch. Do not send legal notices, personal data, or health information through the public GitHub issue tracker.
